Lactulose solution is indicated for the treatment of chronic constipation and as an adjunct in the management of hepatic (portal-systemic) encephalopathy, including the prevention and treatment of episodes of hepatic coma and precoma. In the context of hepatic encephalopathy, Lactulose acidifies the colonic contents to reduce colonic generation of ammonia and facilitates its excretion, reducing blood ammonia levels.
Lactulose is a synthetic sugar that the body cannot digest or absorb. When it reaches the large intestine, it draws water into the bowel by osmosis, softening the stool and making it easier to pass. Gut bacteria also ferment Lactulose, producing mild acids that further soften stools and stimulate bowel movement. In liver disease, this same acidifying effect helps trap and remove ammonia from the gut before it can reach the blood.
Lactulose (a synthetic disaccharide of galactose and fructose) is not hydrolysed by intestinal disaccharidases and reaches the colon intact. Colonic bacteria metabolise lactulose to short-chain fatty acids (lactic, acetic, formic) and CO2, lowering luminal pH and increasing osmotic pressure. The osmotic effect draws water into the colonic lumen, increasing stool water content and promoting peristalsis. In hepatic encephalopathy, the acidic environment converts ammonia (NH3) to the non-absorbable ammonium ion (NH4+), reducing colonic ammonia absorption and lowering systemic blood ammonia levels. It also reduces colonic transit time, decreasing overall ammonia production.
Lactulose was first developed in the 1950s as a synthetic sugar and was initially studied for infant nutrition. Its unique property of being entirely undigested by the human gut led to its use as a safe laxative and later as a cornerstone treatment for hepatic encephalopathy in patients with liver disease.
ZiaLact Solution should be taken as directed by your doctor, usually once or twice daily. It can be mixed with water, juice, or other beverages. Results in constipation may take 24–48 hours — do not increase the dose prematurely. Drink plenty of water throughout the day.

Initial use may cause bloating, gas, stomach cramps, or a feeling of fullness as the gut adjusts. These effects usually subside with continued use. Diarrhoea may occur if the dose is too high. Tell your doctor if any side effect is severe or persistent.
