Zianerve-Forte Tablet is a carefully formulated combination containing Methylcobalamin 1500 mcg, Pregabalin 75 mg, and Nortriptyline 10 mg — three agents that work in concert to relieve neuropathic (nerve) pain, support nerve health, and improve sleep disrupted by chronic pain.
This combination is widely used in the management of diabetic neuropathy, post-herpetic neuralgia, and other chronic nerve pain syndromes, offering a comprehensive approach that addresses both the symptoms and the underlying nerve damage.
This medicine is commonly used for:
Zianerve-Forte is indicated for the management of peripheral neuropathic pain syndromes including diabetic peripheral neuropathy, post-herpetic neuralgia, and idiopathic sensory neuropathy. Methylcobalamin supports axonal regeneration; Pregabalin modulates central sensitisation; and Nortriptyline enhances descending inhibitory pain control and improves comorbid sleep disturbance.
Pregabalin calms overactive nerve signals responsible for the burning and shooting sensations of nerve pain. Methylcobalamin is an active form of Vitamin B12 that nourishes and helps repair damaged nerve fibres. Nortriptyline, used here at a low dose, helps modulate pain signals in the brain and also improves sleep quality — both of which are often impaired in nerve pain conditions.
Pregabalin binds to the alpha-2-delta subunit of voltage-gated calcium channels in the central and peripheral nervous system, reducing the release of excitatory neurotransmitters (glutamate, substance P, noradrenaline) and thereby dampening central sensitisation. Methylcobalamin is the neurologically active form of cobalamin that participates in myelin synthesis, methionine synthase reactions, and axonal transport — supporting remyelination and neuroprotection. Nortriptyline, a secondary amine tricyclic, inhibits the reuptake of noradrenaline and serotonin, augmenting the descending inhibitory pain pathways; its H1-receptor antagonism also contributes to sedation and improved sleep at sub-antidepressant doses.
Methylcobalamin is the only form of Vitamin B12 that can directly cross the blood-brain barrier and is used by neurons — making it the preferred neurological form compared to cyanocobalamin found in most standard B12 supplements.
Zianerve-Forte Tablet is usually taken at night as prescribed by your doctor, since it may cause some drowsiness. Take it regularly as directed — nerve pain management often requires consistent use over a period of time for full benefit.
Always follow the dosage and schedule advised by your doctor — this is general guidance only.
Common side effects include dizziness, drowsiness, mild weight gain, dry mouth, and blurred vision. These are largely from the Pregabalin and Nortriptyline components and tend to ease with continued use. Tell your doctor if any side effect is severe or persistent.
Nerve pain will go away on its own without treatment.
Neuropathic pain is caused by actual nerve damage or dysfunction and rarely resolves without targeted treatment. Early management can prevent the pain from becoming chronic and support nerve recovery.
Antidepressants in this combination mean it is only for depression.
Nortriptyline is included here at a low dose specifically for its well-established analgesic effect on nerve pain pathways — a completely separate mechanism from its antidepressant use at higher doses.
