Cefpodoxime proxetil 100 mg per 5 ml oral suspension is indicated for paediatric patients requiring higher-dose cefpodoxime therapy for susceptible bacterial infections, including acute otitis media, group A streptococcal pharyngitis and tonsillitis, community-acquired pneumonia, acute exacerbations of chronic bronchitis, uncomplicated urinary tract infections, and skin and skin-structure infections. The double-strength formulation is particularly suited for older children or those requiring weight-adjusted doses that would require large volumes of the standard-strength suspension.
Like Ziapodxim, the DS version is absorbed in the gut and converted to active cefpodoxime, which disrupts the bacterial cell wall — causing bacteria to die. The higher concentration means the same antibacterial effect is achieved in a smaller volume of liquid, which is more practical and easier to measure for older children.
Cefpodoxime proxetil is an ester prodrug hydrolysed to active cefpodoxime by intestinal mucosal esterases. Active cefpodoxime binds preferentially to PBP-3 in bacterial cell walls, inhibiting cell wall transpeptidation, leading to autolytic enzyme activation and bactericidal cell death. Its beta-lactamase stability and broad-spectrum activity encompass Streptococcus pneumoniae, Haemophilus influenzae (including ampicillin-resistant strains), Moraxella catarrhalis, Staphylococcus aureus (methicillin-susceptible strains), and major gram-negative uropathogens. Food enhances its oral bioavailability approximately 1.3-fold through prolonged gastric transit.
The letter 'DS' in the name stands for 'Double Strength' — Ziapodxim-DS contains exactly twice the concentration of Cefpodoxime Proxetil (100 mg per 5 ml) compared to the standard Ziapodxim (50 mg per 5 ml), allowing the same dose in a smaller volume.
Ziapodxim-DS Dry Syrup should be given as prescribed by the doctor. Always give it with food or at mealtimes for better absorption. Shake well before each use, and ensure the full course is completed as advised even if symptoms improve.

Fever resolution is a sign of improvement but not complete bacterial eradication. Stopping antibiotics early increases the risk of the infection returning and can promote antibiotic resistance. Always complete the full course.
