Ofloxacin + Metronidazole + Simethicone combination is indicated in mixed gastrointestinal infections caused by susceptible bacteria and anaerobes/protozoa, including amoebiasis, giardiasis, and bacterial gastroenteritis, particularly when bloating and flatulence are concurrent symptoms. Metronidazole targets anaerobic bacteria and protozoa while Ofloxacin covers aerobic gram-positive and gram-negative pathogens; Simethicone provides adjunctive relief of gaseous distension.
Ofloxacin kills susceptible bacteria by blocking their ability to replicate their DNA. Metronidazole disrupts the DNA of anaerobic bacteria and protozoa such as amoeba and giardia, helping to clear protozoal infections. Simethicone reduces the surface tension of gas bubbles in the gut, helping trapped gas pass more comfortably and relieving bloating.
Ofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, preventing DNA replication and repair. Metronidazole, a nitroimidazole, is reduced intracellularly by anaerobic organisms to form reactive intermediates that cause DNA strand breakage and inhibit nucleic acid synthesis, leading to cell death in susceptible anaerobes and protozoa (Entamoeba histolytica, Giardia lamblia, Trichomonas vaginalis). Simethicone is a non-absorbable defoaming agent that decreases surface tension of mucus-entrapped gas bubbles in the GI tract, facilitating their coalescence and expulsion.
