Cofles LS Syrup is indicated in the management of productive cough associated with acute and chronic bronchitis, bronchial asthma, COPD, and other obstructive airway diseases where mucociliary clearance is impaired and bronchospasm is present. Ambroxol provides mucokinetic and mucolytic activity; Guaifenesin acts as an expectorant; Levosalbutamol provides selective beta-2 adrenergic bronchodilation to relieve bronchospasm and facilitate mucus expulsion.

Ambroxol HCl stimulates type II pneumocytes to increase surfactant secretion, depolymerises mucopolysaccharide strands in mucus to reduce viscosity, and enhances mucociliary beat frequency. Guaifenesin acts as an expectorant by increasing the volume of respiratory secretions through irritation of the gastric mucosa and reflex activation of secretory glands, reducing mucus adhesiveness. Levosalbutamol is the pharmacologically active R-enantiomer of salbutamol; it selectively binds beta-2 adrenergic receptors on bronchial smooth muscle, activating adenylate cyclase to raise intracellular cAMP, causing smooth muscle relaxation and bronchodilation with fewer cardiac side effects than the racemic mixture.
Levosalbutamol (the R-enantiomer of salbutamol) was developed specifically to provide bronchodilation with fewer side effects — such as reduced tremors and less heart rate elevation — compared to the older racemic salbutamol formulations, making it a preferred bronchodilator ingredient in combination cough syrups.
