Diclofenac Diethylamine topical gel is indicated for the local symptomatic relief of pain and inflammation in localised forms of soft tissue rheumatism, acute musculoskeletal injuries (sprains, strains, contusions), arthralgia (joint pain), low back pain, and periarticular conditions such as tendinitis and bursitis. Methyl salicylate and menthol act as counter-irritants providing additional symptomatic pain relief through cutaneous receptor modulation.
When Dicrex-Gel is massaged into the skin, the diclofenac is absorbed locally into the underlying tissues, where it reduces the production of prostaglandins — chemicals the body makes in response to injury or inflammation that cause pain and swelling. Methyl salicylate and menthol create a warming and cooling sensation that further helps to soothe pain and discomfort at the surface level.
Diclofenac is a non-selective COX-1 and COX-2 inhibitor that suppresses arachidonic acid conversion to prostaglandins and thromboxanes by blocking cyclooxygenase enzymes, thereby reducing local tissue inflammation and pain. As the diethylamine salt, it exhibits superior transdermal penetration compared with sodium salt formulations, achieving therapeutically relevant sub-cutaneous and synovial concentrations while minimising systemic absorption. Methyl salicylate (a salicylate ester) provides additional COX inhibition and counter-irritant analgesia through activation of TRPV1 thermal receptors. Menthol activates TRPM8 cold receptors, producing cooling analgesia and modulating pain signal transmission. Benzyl alcohol contributes mild local anaesthetic and antimicrobial properties.
Diclofenac as a topical gel can reach therapeutically relevant concentrations in the synovial fluid of joints below the application site — without achieving significant levels in the bloodstream. This makes it a uniquely targeted option for joint pain with a much lower risk of stomach and cardiovascular side effects compared with oral diclofenac.
Dicrex-Gel is for external use only. Apply a small amount to the affected area and massage gently until absorbed. Use as directed by your doctor, typically 3–4 times daily. Wash hands thoroughly after application. Avoid applying over large areas of skin or for prolonged periods beyond what is prescribed.
