Ofloxacin + Ornidazole + Racecadotril combination is indicated in acute secretory diarrhoea of mixed aetiology, including bacterial gastroenteritis, amoebiasis, and giardiasis. Ofloxacin covers susceptible gram-positive and gram-negative bacteria; Ornidazole targets anaerobic bacteria, Entamoeba histolytica, and Giardia lamblia; Racecadotril (acetorphan) acts as an enkephalinase inhibitor to reduce intestinal hypersecretion without affecting gut motility, making it particularly suitable for paediatric infectious diarrhoea.
Ofloxacin targets and kills bacteria causing the gut infection by interfering with their DNA copying process. Ornidazole works similarly against anaerobic bacteria and protozoa like amoeba. Racecadotril reduces the excessive fluid that the gut secretes during infection — helping stools become less watery — without slowing down normal bowel movement, so the infection can still clear naturally.
Ofloxacin inhibits bacterial DNA gyrase and topoisomerase IV causing lethal DNA strand breaks. Ornidazole, a nitroimidazole, is selectively reduced in anaerobic organisms to cytotoxic intermediates that damage DNA and inhibit nucleic acid synthesis. Racecadotril (prodrug of thiorphan) inhibits enkephalinase (neutral endopeptidase, neprilysin) at the intestinal brush border, preventing degradation of endogenous enkephalins, which tonically inhibit adenylyl cyclase in enterocytes, thereby reducing hypersecretion of water and electrolytes into the intestinal lumen without altering intestinal motility.
Racecadotril, one of the key ingredients in this combination, works differently from most anti-diarrhoeal drugs — it does not slow the bowel but instead reduces the gut's tendency to secrete excess fluid. This means the gut can still naturally clear the infection while diarrhoea is reduced.
Oflozia-OZR Suspension should be taken as directed by your doctor. Shake the bottle well before each use. It is typically taken before meals. Follow the complete course as prescribed, even if diarrhoea resolves sooner.

Common side effects may include mild nausea, metallic taste, headache, or occasional dizziness. Racecadotril is generally very well tolerated. These effects are usually mild and temporary. Tell your doctor if any side effect is severe or persistent.
Anti-diarrhoeal medicines should be avoided because they trap the infection inside.
Racecadotril works by reducing fluid secretion, not by stopping bowel movements. It does not trap pathogens, so it is considered safe to use alongside antimicrobials for infectious diarrhoea.
Once diarrhoea stops, the infection is fully cleared.
Symptomatic improvement does not always mean microbiological clearance. Completing the full prescribed antibiotic course is essential to fully eliminate the infection and prevent recurrence.
